Same issues. Pulled the covers off the motor. Upon inspection they found excessive play in the fuel rack and worn lobes on the cam. Replaced with reman long block, 5 new injectors, new drivers, new rack and new hard lines. Also new fuel shut off solenoid. Called and said it's fixed.
